Volunteer Registration Form - an example for single or multiple roles
A couple of years ago, I worked with our Safeguarding team to establish a system of logging all our volunteers and the different roles they did, so that we could then make sure the right people had the right checks needed to keep everyone safe. (This is the nitty gritty love of God in action)
We identified an issue that might resonate with you. One person often has a number of voluntary roles within one church, and completing a separate form for each role seemed inefficient.
So we created a single form that has capacity to log the multiple roles that someone does. We created our form on our Church Management platform and sent the link to people, but I'm offering you a Word version of it (here) which you can then edit or copy & paste into a platform version.
The sections included in the form:
- Contact information
- Roles (we made room for 4 roles)
- DBS Check info
- References
- Additional Safety information (emergency contact, allergies etc)
- Consents
Once the form was completed, our Safeguarding Officer and DBS Admin (both volunteers) were able to ask for references, and get the ball rolling on training and DBS checks.
Two years on from implementation and we have founded we have a manageable system that enables safer recruitment of volunteers and helps us to easily spot what we need to be doing.
